Drug Interactions

clopidogrel and esomeprazole

MajorAvoid this combination where possible.

Mechanism
Pharmacokinetic interaction: esomeprazole inhibits the hepatic cytochrome P450 enzyme CYP2C19, which is required for the conversion of clopidogrel (a prodrug) into its active metabolite. Reduced formation of the active metabolite diminishes clopidogrel's antiplatelet effect.
What it means clinically
Concurrent use can lead to reduced inhibition of platelet aggregation, increasing the risk of cardiovascular events such as stent thrombosis, myocardial infarction, and stroke in patients with acute coronary syndromes or after percutaneous coronary intervention.
What to do
Avoid co-prescription when possible. If a proton pump inhibitor is needed, consider using pantoprazole, which has minimal CYP2C19 inhibition, or an alternative antiplatelet agent such as ticagrelor or prasugrel. If the combination is considered necessary, monitor patients closely for signs of cardiovascular events and assess platelet function if available.
Evidence
Well-established through pharmacokinetic studies and clinical outcome trials. Regulatory warnings (e.g., FDA) advise caution, and guidelines recommend limiting the use of PPIs that inhibit CYP2C19 with clopidogrel.
