Drug Interactions

rivaroxaban and clopidogrel

MajorAvoid this combination where possible.

Mechanism
Pharmacodynamic interaction: rivaroxaban inhibits factor Xa, reducing thrombin generation, while clopidogrel irreversibly inhibits platelet P2Y12 receptors, preventing platelet activation and aggregation. The combined effect on the coagulation cascade and platelet function results in additive impairment of haemostasis.
What it means clinically
Co-administration of rivaroxaban and clopidogrel substantially increases the risk of major bleeding, including intracranial haemorrhage and fatal bleeding, compared with either agent alone. This combination is often used in certain cardiac conditions, such as atrial fibrillation with acute coronary syndrome or after percutaneous coronary intervention, where the benefit may outweigh the risk in selected patients.
What to do
Avoid concomitant use unless the clinical benefit outweighs the bleeding risk. If the combination is necessary, use the lowest effective doses of both agents, monitor the patient closely for signs of bleeding, and consider using a proton pump inhibitor for gastroprotection. Regularly review the ongoing need for dual therapy.
Evidence
The interaction is well established based on pharmacokinetic studies and clinical trials, including those in patients with atrial fibrillation undergoing PCI. The risk of bleeding is dose-dependent and increased with longer duration of combination therapy. Guidelines recommend careful patient selection and limited duration of dual therapy to reduce bleeding risk.
