Drug Interactions

fluoxetine and clopidogrel

ModerateUsually manageable, but it must be recognised and monitored.

Mechanism
Fluoxetine inhibits CYP2C19, the enzyme responsible for the bioactivation of clopidogrel to its active metabolite. This reduces the formation of the active antiplatelet substance, potentially decreasing clopidogrel's efficacy. Fluoxetine also inhibits serotonin reuptake, which can impair platelet aggregation and increase bleeding risk when combined with antiplatelet agents.
What it means clinically
The clinical impact is variable. Reduced clopidogrel activation may increase the risk of cardiovascular events, such as stent thrombosis or myocardial infarction. However, some patients may also experience an increased risk of bleeding due to the additive antiplatelet effect of fluoxetine. Routine co-prescription is generally avoided or monitored, especially in patients with cardiovascular risk factors.
What to do
If concomitant use is necessary, consider an alternative SSRI that is less likely to inhibit CYP2C19 (e.g., sertraline, citalopram), or use an alternative P2Y12 inhibitor (e.g., prasugrel or ticagrelor) if clinically appropriate. Monitor for signs of bleeding and thrombotic events. Where possible, prescribe the lowest effective dose of fluoxetine and regularly review the combination.
Evidence
Evidence is based on pharmacokinetic studies and case reports. The interaction is recognised in summary of product characteristics and drug interaction references, but clinical significance is not consistently demonstrated in outcome trials. No direct randomised controlled trials have been conducted to assess the clinical impact of this combination.
