Drug Interactions

dabigatran and clopidogrel

MajorAvoid this combination where possible.

Mechanism
Pharmacodynamic interaction. Dabigatran directly inhibits thrombin, while clopidogrel inhibits platelet activation via the P2Y12 receptor. Both drugs impair haemostasis through different pathways, leading to an additive effect on bleeding risk.
What it means clinically
The combination significantly increases the risk of major bleeding, including intracranial haemorrhage and gastrointestinal bleeding. The risk is further elevated in patients with renal impairment, advanced age, or those taking other drugs that affect haemostasis.
What to do
Avoid co-prescription unless the benefit clearly outweighs the risk, such as in specific cardiac indications. If combined, use the lowest effective doses, monitor closely for signs of bleeding, and consider gastroprotective therapy. Perform regular full blood counts and renal function tests.
Evidence
Well established in clinical practice guidelines and product information. The interaction is based on pharmacodynamic principles and consistent clinical observations of increased bleeding events.
