Drug Interactions

apixaban and clopidogrel

MajorAvoid this combination where possible.

Mechanism
Both drugs impair haemostasis through different pathways. Apixaban inhibits factor Xa, reducing thrombin generation, while clopidogrel irreversibly inhibits P2Y12 receptors on platelets, preventing platelet activation and aggregation. When combined, these effects add to produce an increased risk of bleeding.
What it means clinically
Co-administration of apixaban and clopidogrel significantly increases the risk of bleeding, including major and minor bleeding events. Patients may experience gastrointestinal bleeding, intracranial haemorrhage, or bleeding at puncture sites. The risk is particularly elevated in patients with additional risk factors such as advanced age, renal impairment, or concurrent use of other anticoagulants or NSAIDs.
What to do
Avoid the combination unless there is a clear indication for dual antithrombotic therapy as recommended by guidelines. If co-prescription is necessary, use the lowest effective doses and the shortest duration. Monitor closely for signs of bleeding (e.g., dark stools, haematuria, bruising) and check haemoglobin and haematocrit periodically. Consider omeprazole or other gastroprotective agents. Assess renal function and adjust apixaban dose accordingly.
Evidence
The interaction is well established. Clinical trials and registry data have shown increased bleeding rates with combination therapy compared to either agent alone. Guidelines from major cardiology societies recommend careful risk stratification and limited duration for dual therapy.
