Drug Interactions

warfarin and prednisolone

MajorAvoid this combination where possible.

Mechanism
Prednisolone may enhance the anticoagulant effect of warfarin through both pharmacokinetic and pharmacodynamic mechanisms. Pharmacokinetically, corticosteroids such as prednisolone may induce hepatic enzymes and alter the metabolism of vitamin K-dependent clotting factors, potentially increasing warfarin's activity. Pharmacodynamically, corticosteroids increase the risk of gastrointestinal mucosal damage and bleeding, which can be additive to the anticoagulant effect of warfarin. The precise pathway is not fully defined, but clinical evidence supports an interaction of clinical relevance.
What it means clinically
Co-administration of warfarin and prednisolone can lead to a significant increase in the international normalised ratio (INR) and an elevated risk of bleeding, particularly gastrointestinal haemorrhage. The effect is variable and may be more pronounced with high doses of prednisolone or in patients with other risk factors for bleeding. Conversely, upon discontinuation of prednisolone, the anticoagulant effect of warfarin may decrease, resulting in subtherapeutic INR and risk of thromboembolism. Close monitoring is required during both initiation and cessation of prednisolone therapy.
What to do
Monitor the INR frequently, particularly within the first 2-3 days after initiating or stopping prednisolone, and adjust the warfarin dose accordingly. Consider adding a gastroprotective agent (e.g., a proton pump inhibitor) in patients at increased risk of gastrointestinal bleeding. Advise patients to report any signs of bleeding, such as easy bruising, dark stools, or haematuria.
Evidence
The interaction is well-established through clinical case reports and observational studies. It is more clearly documented with high-dose corticosteroid therapy, and monitoring recommendations are widely accepted in clinical practice. However, the magnitude of INR change can vary between individuals, and formal prospective studies are limited.
