Drug Interactions

warfarin and clarithromycin

MajorAvoid this combination where possible.

Mechanism
Clarithromycin inhibits CYP3A4, the enzyme responsible for the metabolism of the less potent R-isomer of warfarin. This inhibition reduces the clearance of warfarin, leading to increased plasma concentrations. Additionally, clarithromycin may reduce the production of vitamin K by gut bacteria, further enhancing the anticoagulant effect.
What it means clinically
The combined effect increases the anticoagulant action of warfarin, potentially leading to a significant rise in the international normalised ratio (INR) and an increased risk of serious or life-threatening haemorrhage. This can occur within a few days of starting clarithromycin.
What to do
Avoid co-prescription if possible. If no suitable alternative antibiotic is available, closely monitor the INR, particularly during the first week of concurrent therapy, and adjust the warfarin dose as necessary. Continue monitoring for a few days after stopping clarithromycin.
Evidence
This interaction is well established and is routinely listed in major drug interaction databases. Numerous case reports and clinical studies have documented elevated INR and bleeding events.
