Drug Interactions

omeprazole and dabigatran

ModerateUsually manageable, but it must be recognised and monitored.

Mechanism
Omeprazole raises gastric pH, which can reduce the dissolution and absorption of dabigatran etexilate and lower dabigatran plasma concentrations by up to about 30%; the primary mechanism is reduced gastric acidity rather than P-gp inhibition.
What it means clinically
Increased dabigatran exposure can raise the risk of bleeding events, particularly gastrointestinal bleeding. The effect is usually modest and may be clinically relevant in patients with renal impairment or other bleeding risk factors.
What to do
Use with caution. Monitor patients for signs and symptoms of bleeding. Consider an alternative proton pump inhibitor with less P-gp inhibition (e.g., pantoprazole) if feasible. If coadministration is necessary, no routine dose adjustment is recommended, but periodic clinical assessment and renal function monitoring are advised.
Evidence
Established from pharmacokinetic studies showing increased dabigatran AUC and Cmax when coadministered with omeprazole. Clinical trials have not shown a significant increase in major bleeding, but some observational studies suggest a higher risk of bleeding.
