The scenario

A 34-year-old woman presents with a 3-day history of dysuria, urinary frequency and urgency. She reports no fever, no flank pain, and no vaginal discharge. Her past medical history is unremarkable, and she takes no regular medicines. She has no known drug allergies. She is not pregnant. She weighs 65 kg. On examination, she is afebrile (temperature 36.8°C), blood pressure 118/76 mmHg, pulse 72 bpm. Abdominal examination is soft with no suprapubic tenderness. Renal angle is non-tender. Urine dipstick shows 3+ leucocyte esterase and 2+ nitrites, with a trace of blood. Serum creatinine is 0.8 mg/dL and eGFR is 95 mL/min/1.73 m². Liver function tests are normal. A urine culture has been sent, but the result is not yet available. Task: Write a prescription for an appropriate oral antibiotic, including dose, duration and counselling points.

What gets marked

  • Whether you chose an indicated drug, and whether you avoided the contraindicated ones.
  • Dose, route, frequency and duration against the range for this patient.
  • Renal and hepatic dose adjustment where this patient's numbers demand it.
  • Allergy cross-reactivity and interactions with the medicines already on the chart.
  • The instructions and monitoring you write, not only the drug you pick.

8 scoring rules apply to this scenario. A single critical error fails the attempt whatever the score.
