The scenario
Patient B-217, a 58-year-old woman, attends a routine diabetes review. She has type 2 diabetes diagnosed 8 years ago, complicated by albuminuria, and essential hypertension. Current medicines: metformin 500 mg twice daily, amlodipine 5 mg once daily, and ibuprofen 400 mg three times daily for knee osteoarthritis. She has no known drug allergies. She does not smoke. On examination: blood pressure 152/92 mm Hg, pulse 76 beats per minute and regular, body mass index 28 kg/m². Weight 70 kg. Urinalysis shows protein:creatinine ratio 8 mg/mmol. Blood tests: sodium 138 mmol/L, potassium 4.4 mmol/L, creatinine 1.1 mg/dL (eGFR 52 mL/min/1.73 m²), HbA1c 7.8%. She is not pregnant. The clinician decides to start an ACE inhibitor to control blood pressure and reduce albuminuria. Prescribe the appropriate medicine, and state what monitoring is required and which of her current medicines should be stopped or avoided.
What gets marked
- Whether you chose an indicated drug, and whether you avoided the contraindicated ones.
- Dose, route, frequency and duration against the range for this patient.
- Renal and hepatic dose adjustment where this patient's numbers demand it.
- Allergy cross-reactivity and interactions with the medicines already on the chart.
- The instructions and monitoring you write, not only the drug you pick.
8 scoring rules apply to this scenario. A single critical error fails the attempt whatever the score.
