The scenario

Patient B-217, a 32-year-old woman, is 18+3 weeks pregnant with her first child. She presents to the antenatal clinic with a two-day history of frontal headache and visual blurring. She has no significant past medical history and takes no regular medications. There is no family history of hypertension. Her blood pressure is 160/110 mmHg on two consecutive readings taken 15 minutes apart. Her heart rate is 92 bpm, respiratory rate 16 per minute, temperature 37.0 degrees Celsius. Urinalysis shows no protein and no blood. Blood tests reveal haemoglobin 112 g/L, platelets 220 x10^9/L, alanine aminotransferase 20 U/L, serum creatinine 55 micromol/L (0.62 mg/dL), estimated GFR 120 mL/min/1.73m2, and uric acid 250 micromol/L. Obstetric ultrasound confirms a viable singleton fetus with normal growth and liquor volume. She has no peripheral oedema, and her blood pressure is confirmed on repeat measurement. The clinical picture is consistent with hypertension in pregnancy.

What gets marked

  • Whether you chose an indicated drug, and whether you avoided the contraindicated ones.
  • Dose, route, frequency and duration against the range for this patient.
  • Renal and hepatic dose adjustment where this patient's numbers demand it.
  • Allergy cross-reactivity and interactions with the medicines already on the chart.
  • The instructions and monitoring you write, not only the drug you pick.

8 scoring rules apply to this scenario. A single critical error fails the attempt whatever the score.
