The scenario
A 4-year-old child presents to the emergency department with a 2-day history of fever, earache, and irritability. The mother reports the child has been pulling at the left ear and has had reduced appetite. There is no vomiting or diarrhoea. The child is fully immunised, has no known drug allergies, and takes no regular medicines. On examination, temperature is 38.9°C, heart rate 110 beats per minute, respiratory rate 24 breaths per minute, and oxygen saturation 98% on room air. Otoscopy reveals a bulging, erythematous left tympanic membrane with reduced mobility on pneumatic otoscopy. The right ear is normal. There is no mastoid tenderness or swelling. Neurological examination is unremarkable. The child weighs 18 kg. No laboratory investigations are performed as the presentation is consistent with acute otitis media. The child is otherwise well and has no significant medical history. Task: Write a prescription for this patient, including appropriate antibiotic therapy and analgesic for pain. State the drug, dose, frequency, and duration for each.
What gets marked
- Whether you chose an indicated drug, and whether you avoided the contraindicated ones.
- Dose, route, frequency and duration against the range for this patient.
- Renal and hepatic dose adjustment where this patient's numbers demand it.
- Allergy cross-reactivity and interactions with the medicines already on the chart.
- The instructions and monitoring you write, not only the drug you pick.
8 scoring rules apply to this scenario. A single critical error fails the attempt whatever the score.
