Drug Interactions

carvedilol and verapamil

MajorAvoid this combination where possible.

Mechanism
Additive negative chronotropic, dromotropic, and inotropic effects on the heart. Additionally, verapamil can inhibit the hepatic metabolism of carvedilol via CYP2D6, raising carvedilol plasma concentrations.
What it means clinically
Potential for severe bradycardia, AV block, hypotension, and worsening heart failure, especially in patients with underlying conduction disease or reduced left ventricular function.
What to do
Avoid coadministration, particularly in patients with heart failure or conduction abnormalities. If necessary, use cautiously with baseline ECG and monitoring of heart rate, blood pressure, and signs of heart failure. Reduce doses as needed.
Evidence
Well documented in clinical literature and standard interaction databases. Evidence is based on case reports, pharmacodynamic studies, and expert consensus.
