Drug Interactions

carbamazepine and simvastatin

MajorAvoid this combination where possible.

Mechanism
Carbamazepine is a potent inducer of cytochrome P450 3A4 (CYP3A4). Simvastatin is extensively metabolised by CYP3A4 to its active metabolites. Induction of CYP3A4 by carbamazepine markedly increases the first-pass metabolism of simvastatin, leading to substantially reduced plasma concentrations of simvastatin and its active metabolites. This is a pharmacokinetic interaction at the level of metabolism.
What it means clinically
The effect is a significant reduction in the lipid-lowering efficacy of simvastatin, potentially leading to inadequate control of hypercholesterolaemia and increased cardiovascular risk. Conversely, if carbamazepine is discontinued while simvastatin dose is unchanged, simvastatin concentrations can rise, increasing the risk of myopathy and rhabdomyolysis.
What to do
Avoid the combination if possible. If a statin is needed, consider a statin not metabolised by CYP3A4, such as pravastatin or rosuvastatin. If the combination is unavoidable, monitor lipid levels regularly and adjust the simvastatin dose accordingly, with careful consideration of the risk of myopathy when carbamazepine is stopped. Patients should be counselled to report unexplained muscle pain, tenderness, or weakness.
Evidence
Well-established. Multiple studies and case reports demonstrate a marked reduction in simvastatin exposure with carbamazepine coadministration, and this interaction is listed in standard references.
