The scenario

Patient B-217 is a 35-year-old woman who presents with worsening fatigue and breathlessness on exertion for 2 months. She reports heavy menstrual bleeding for the past 6 months, with periods lasting 8 to 10 days, requiring pad changes every 2 hours, and occasional large clots. She has a history of hypothyroidism and takes levothyroxine 100 micrograms once daily. She has no known drug allergies. Her weight is 65 kg. On examination, she appears pale, heart rate is 102 bpm, blood pressure 110/68 mmHg, and the rest of the examination is unremarkable. Investigations: haemoglobin 89 g/L, MCV 72 fL, ferritin 6 micrograms/L, serum iron 5 micromol/L, total iron-binding capacity 80 micromol/L, transferrin saturation 6%, white cell count 6.8 x 10^9/L, platelets 410 x 10^9/L. Renal and liver function are normal. She is not pregnant. Write a prescription for the management of her anaemia. Include the drug, dose, frequency, and any relevant counselling. Also state when you would recheck her blood counts.

Your task

Heavy menstrual bleeding for 6 months, fatigue, dyspnoea on exertion.

What gets marked

  • Whether you chose an indicated drug, and whether you avoided the contraindicated ones.
  • Dose, route, frequency and duration against the range for this patient.
  • Renal and hepatic dose adjustment where this patient's numbers demand it.
  • Allergy cross-reactivity and interactions with the medicines already on the chart.
  • The instructions and monitoring you write, not only the drug you pick.

8 scoring rules apply to this scenario. A single critical error fails the attempt whatever the score.
